Appointments

1. Review your doctor's in-clinic schedule to review current appointment times.
2. Complete and print our adult ()or pediatric () medical history form.
3. Call 723-6963 on the day you would like to be seen and we will schedule your visit.
Please arrive 15 minutes prior to appointment to complete pre-visit registration.

Travel Advice Planner

We offer travel-related advice and immunizations only to established patients of SFM. Please review your itinerary and the travel medicine Information in our Educational Resources. Then you may complete the travel form below and fax to your doctor at 650/487-7758. Your physician will review this information and we will contact you to arrange the appointment - please allow us 72 hours to review and obtain the necessary approvals by your health plan before scheduling this appointment
