Patient Guide

Access, Hours, Maps
We are open Monday through Friday, from 8 to 12 noon and 1:00 to 5 p.m. Our office is on the third floor of the Hoover Pavilion, the 4 story art deco building located across from Bloomingdale's at the Stanford Shopping Center, at 211 Quarry Road.    We are on the Marguerite "A" shuttle route, which runs every 15 minutes between the Medical Center and Hoover.  Paid patient parking is located in the lot right in front of the building. This costs $1.50 per hour and can ONLY be paid with exact change or by credit card.  Please be sure to park only in the area reserved for "clinic patients."

It is important to make an appointment before coming in so you can be seen promptly and out of courtesy for other, scheduled patients. Please phone (650)723 6963.

After 5 p.m. and on weekends and holidays there is someone on call to handle urgent medical problems. If you call the clinic number (723-6963) after office hours you will be automatically forwarded to our answering service, which will locate the person on call for our group. If you have a child with an after hours urgent medical problem, please call our clinic number (723 6963) to access our Pediatric After Hours nurses and clinic.

In addition to seeing patients, several of us spend time teaching, administrating and doing research. If your personal physician is not available, one of the other providers will be able to take care of you.

Choosing Your Physician
Your selection of a physician is very important and should be done as soon as you enroll with the group. Please note that some physicians may not have openings for new patients at all times. If you were previously assigned to an SFM physician or picked a physician but have not yet made an appointment, we will try to accommodate your choice


Who We Are

Emergency Care
Emergency Room Visits outside clinic hours no longer require pre-authorization by your doctor (by California State Law) but should be reported to your physician within 24 hours following your visit. Be aware of your health plan limitations -- unless you require 911 emergency transport, some health plans require you to obtain all after-hours emergency care at Stanford ER if you have a primary care physician at Stanford. A physician is on-call after hours to coordinate any emergency care needs which arise after-hours and can be reached at the 24 hour number - 650-723-6963.

Stanford Family Medicine do not renew routine prescriptions or narcotic prescription after-hours as your records are not available for review by the on-call physician. Please leave a voice mail message with your pharmacy or practice coordinator so that your records can be verified the next day and your prescription refilled during usual business hours.

Lab and Test Results

All normal lab and test results are mailed to you within two weeks of the physicians’ receiving the results. Abnormal results or those of an urgent nature will be phoned directly to you as soon as the results are available.

Prescriptions

Routine prescription renewals: Please contact your pharmacy directly and they will utilize our fax-back service to renew your prescription - 650/498-7750 (Fax))

New prescriptions: contact your physician/practice coordinator directly.

Using Email To Reach SFM

Stanford Family Medicine's group in partnership with RelayHealth is offering online access using the RelayHealth messaging service. We would like you to consider using this method of communication to reach your physician when an immediate answer is not required.

Health Insurance Issues

The Stanford Hospital and Clinics contract with many insurance companies. Since each plan can vary between employers, it is extremely important that you understand the benefits and limitations of your health care plan. This information can be obtained by calling your benefits office at your place of work or the health insurance carrier directly.

Please contact your practice coordinator and physician directly for help with referrals and authorizations.
